Entrance. Through the portal of Blackrock Spire, high in Blackrock Mountain. Inside, the Seal of Ascension opens the door to the left of the entrance.
The loot list follows the dungeon journal that Blizzard made for Blackrock Spire in Classic, and only items that exist in the Forever data appear as cards. Three of the 40 items are in that data: Gyth’s Skull from Warchief
Rend Blackhand, and Brigam Girdle and Draconian Deflector from General Drakkisath. Whether Blizzard changes the rest for Forever or those items simply are not in the data yet, is not known.
These drops from Classic are not in the Forever data yet:

Where do you get the Seal of Ascension?
From the quest Seal of Ascension in Lower Blackrock Spire, which needs three gemstones from bosses there.
